What is this survey about?
This survey is about your experience with you or your child's health plan. You were randomly selected from eligible members of the health plan to evaluate the care received. The data will help the health plan to improve the quality of care they provide and will help other consumers make informed decisions when choosing a health plan.
Who is conducting/sponsoring this survey?
This survey is sponsored by your health plan and is part of a national project by the National Committee for Quality Assurance (NCQA), which is a private, non-profit organization that collects information about the quality of health plans. The Center for the Study of Services (CSS) is a research organization hired by your health plan to help conduct this survey.
How was I selected?
You were randomly selected from everyone enrolled in you or your child's health plan. The health plan does not know which members were selected to participate in this survey.
How will my answers be used?
The survey is designed to evaluate the care members receive from their health plan. The data will help health plans improve the quality of care they provide. It will also help consumers make informed decisions when choosing a health plan.
Who will see my answers?
Your answers will be kept absolutely confidential and your completed survey will not be seen by anyone other than the research staff at the Center for the Study of Services (CSS). Your answers will be grouped with the answers of all other survey participants from your health plan. Published reports will not show any names or individual answers. This online survey is on CSS's secure website, and your name will not be shared with your health plan, provider, or anyone else.
Do I have to complete the survey?
Your participation is voluntary and there are absolutely no penalties for not participating. But, it is a very important survey and your answers will help improve the quality of care provided by your health plan. Your answers to the survey will have no effect on your benefits, and once you begin the survey, you may skip any questions you do not wish to answer.
How long does it take to complete the survey?
The survey usually takes less than 20 minutes.
Where can I send other comments about my plan?
If you have a comment or question about your health plan this is not addressed in this survey, please contact the health plan itself. You may wish to contact the health plan’s customer service line at the number listed on your health plan ID card. CSS is an independent company administering the survey on behalf of health plans, and does not collect or communicate anything other than the combined, summarized results of this survey to your health plan.
I'm having trouble accessing/viewing the survey. What should I do?
The following are potential solutions:
- If the page has not fully loaded, click on your browser's "refresh" button.
- Close out of your browser and return to the survey page. Your browser will save your progress.
- Upgrade your browser to the most recent version.
- Make sure your browse screen is big enough by maximizing your browser window. You can also use your mouse to move the scroll bar at the bottom of your browser window to move right and left.
- Close other applications that may be running on your computer.
